Listening Hands® Care Services, LLC

Listening Hands LLC is a Practice with values rooted in compassion and universal love. We provide private care coordination as well as nurse consulting services. Our services help patients and families navigate the healthcare system in order to establish a personalized & integrative plan of care. We advocate for patients, collaborate with providers, and assist with securing healthcare services to ensure continuity of care. 

 

We believe in a holistic approach to advocate for patients and help them to understand why follow through with their healthcare plan is important for overall wellness. 

 

This organization was founded on the fact that a personalized, and culturally sensitive plan of care creates a healing environment and leads to better outcomes for the patient.  We believe that health is attributed to one understanding the healthcare system and how to follow their specialized plan of care. 

 

We, at Listening Hands LLC, also thoroughly understand that all things in life are connected; just as the branches of a tree connect to the trunk; and the roots connect to the earth. The chain of Healthcare should be connected from prevention, to hospital admissions, to discharge, as well as home care.  The break in healthcare, where patients are no longer caring for themselves efficiently, is usually due to the patient not understanding the plan of care, not agreeing with the plan of care, and not having the proper resources to care for themselves. 

 

Listening Hands LLC provides a listening ear & a high level of expertise in care coordination/case management to help patients secure the resources to care for themselves effectively.
